Requirements to Apply

Candidates must meet the following requirements to be considered:

• Registered Dietitian (RD) through the Commission on Dietetic Registration
• Current Florida Dietitian/Nutritionist license
• Fluent bilingual English/Spanish required
• Minimum one year of experience working directly with a frail or elderly population required
• Current ServSafe Certification required
• Current CPR/BLS required prior to start
• Clinical nutrition experience
• Experience within PACE, senior care, geriatric care, adult day health, skilled nursing, or a similar interdisciplinary healthcare environment strongly preferred
• Must be comfortable providing hands-on support to elderly participants and working beyond traditional dietitian responsibilities when needed

Position Summary

Talent Strategies is seeking a compassionate, hands-on Clinical Dietitian for a PACE (Program of All-Inclusive Care for the Elderly) program in Hialeah, Florida.

This is not a traditional desk-based dietitian position. The Clinical Dietitian is an active member of the interdisciplinary care team and works directly with frail and elderly participants to support their nutritional needs, overall health, safety, and quality of life.

We’re looking for someone who is equally comfortable completing clinical assessments and documentation and rolling up their sleeves to help participants during the day. This may include assisting during meals, helping participants who need support with eating, overseeing food service and inventory, working with catering/meal vendors, and stepping in wherever needed to support the participants and team.

The right person will bring a combination of clinical expertise, empathy, flexibility, organization, and genuine passion for serving older adults.

Key Responsibilities

• Complete comprehensive nutritional assessments and reassessments for PACE participants
• Develop individualized nutrition plans as part of each participant’s overall plan of care
• Participate actively in interdisciplinary team (IDT) meetings and care planning
• Monitor participant weight, nutritional intake, hydration, swallowing or chewing concerns, medical conditions, skin integrity, and other factors affecting nutritional health
• Provide nutrition counseling and education to participants and caregivers
• Develop monthly meal plans consistent with nutritional and program requirements
• Oversee meal service and ensure appropriate portions, therapeutic diets, cleanliness, and food-service standards
• Provide hands-on assistance and support to participants during meals when needed
• Monitor food purchasing, supplies, and inventory
• Coordinate with catering and contracted meal-service vendors to ensure participant dietary needs are met
• Supervise and support dietary aides
• Maintain accurate clinical documentation and communicate changes in participant status to the interdisciplinary team
• Participate in quality and performance-improvement activities
• Work collaboratively with nursing, medical, social services, rehabilitation, and other members of the PACE care team
• Step in to support participants and the center as needed in a collaborative, hands-on environment
