Pay
$63,000 – $73,000 annually
Compensation is determined based on a combination of experience, certifications, interview outcome, and final decision between the client and candidate.
Requirements to Apply
- Master’s Degree in Social Work (MSW) from an accredited institution
- Minimum one (1) year of social work experience required
- Minimum one (1) year of experience working with the geriatric population (ages 55+) required
- Healthcare experience preferred
- Fluent Bilingual Spanish or Bilingual Korean required
- Valid California Driver’s License required
- Current CPR/BLS and First Aid certifications required
Job Summary
Talent Strategies is partnering with a respected healthcare organization supporting a PACE (Program of All-Inclusive Care for the Elderly) program serving seniors throughout the San Fernando Valley.
We are seeking a compassionate and mission-driven MSW Social Worker to join an interdisciplinary healthcare team dedicated to helping older adults remain safely in their homes and communities while receiving comprehensive medical, social, and supportive services.
The Social Worker serves as an integral member of the Interdisciplinary Team (IDT), providing psychosocial assessments, care planning, counseling, case management, advocacy, and resource coordination for participants and their families.
Responsibilities
- Participate in Interdisciplinary Team (IDT) meetings and participant care planning
- Complete psychosocial assessments, reassessments, and ongoing participant evaluations
- Conduct participant home visits as needed
- Maintain timely and accurate documentation within participant records
- Provide individual, family, and group counseling services
- Connect participants and caregivers with community resources and supportive services
- Coordinate care and advocate on behalf of participants and families
- Support care transitions and disenrollment processes when needed
- Collaborate with healthcare professionals to address social, emotional, cultural, and family dynamics impacting participant care
- Participate in program development, quality improvement initiatives, and community outreach activities
- Promote participant-centered care and exceptional customer service
