Talent Strategies is recruiting a Social Worker (MSW) in Hialeah, FL for a full-time, onsite opportunity supporting a PACE (Program of All-Inclusive Care for the Elderly) program serving frail and elderly adults.

This Social Worker MSW position is ideal for a compassionate healthcare social worker who enjoys working directly with older adults, families, and caregivers. As part of an interdisciplinary healthcare team, the Social Worker will help address the social, emotional, behavioral, environmental, and practical challenges that can affect a participant’s health, independence, safety, and overall quality of life.

The role combines psychosocial assessment, counseling, case management, care coordination, advocacy, community resources, and family support within a highly collaborative senior-care environment.

LCSW licensure is not required for this position.

Social Worker MSW Requirements

Candidates must meet the following requirements to be considered:

  • Master’s Degree in Social Work (MSW) from a CSWE-accredited school required
  • Bilingual English/Spanish required
  • Minimum 1 year of experience working directly with a frail or elderly population required
  • Current BLS/CPR certification required prior to start
  • Two years of social work experience working as part of an interdisciplinary team in a hospital, nursing home, community-based program, or similar healthcare setting preferred
  • Experience within PACE, geriatric care, senior care, skilled nursing, adult day health, or a similar interdisciplinary healthcare environment strongly preferred
  • Experience with psychosocial assessments, counseling, care coordination, case management, and community resources
  • Comfortable working directly with older adults, families, caregivers, and an interdisciplinary healthcare team

About the Social Worker MSW Position

The Social Worker is an integral member of the PACE interdisciplinary care team, working directly with frail and elderly participants and their families to identify and address psychosocial needs that may affect their ability to remain healthy, safe, independent, and engaged in their community.

The Social Worker completes comprehensive psychosocial assessments, participates in individualized care planning, provides counseling and emotional support, assists families through complex situations, coordinates community resources, and advocates for participant needs.

This is a highly interactive social work position for an MSW who genuinely enjoys building relationships with older adults and helping participants and families navigate the medical, behavioral, financial, housing, family, and social challenges that can accompany aging.

Social Worker MSW Responsibilities

  • Complete comprehensive psychosocial assessments and reassessments for PACE participants
  • Assess social support systems, cognitive status, mental health, family dynamics, living environment, and other psychosocial needs
  • Participate actively in interdisciplinary team (IDT) meetings and comprehensive participant care planning
  • Develop and contribute to individualized plans of care
  • Provide counseling, emotional support, education, and ongoing social work services to participants and families
  • Support participants and caregivers through issues related to aging, dementia, chronic illness, grief and loss, changing family roles, and end-of-life needs
  • Facilitate participant and family meetings and assist with complex family or care-planning situations
  • Coordinate services with participants, caregivers, healthcare providers, facilities, and community agencies
  • Connect participants and families with appropriate community resources and government programs
  • Assist with housing resources, respite care, assisted living, facility placement, and other supportive-service needs
  • Assist eligible participants with SSI/SSDI and other benefit-related resources
  • Support participants and families with advance directives and healthcare planning
  • Assist with transitions of care and changes in participant living arrangements
  • Advocate for participant safety, independence, dignity, and quality of life
  • Recognize and appropriately escalate concerns involving abuse, neglect, exploitation, or participant safety
  • Maintain timely and accurate social work documentation
  • Participate in quality and performance-improvement activities
  • Collaborate closely with medical, nursing, rehabilitation, dietary, behavioral health, and other members of the interdisciplinary care team

Compensation & Schedule

Salary: $73,000–$94,000 annually

Final compensation will be determined based on experience, qualifications, interview outcome, and the final agreement between the client and candidate.
