PACE PROGRAM (Program for All Inclusive Care for the Elderly)
Location: Redondo Beach, CA
Schedule: Full Time | Monday – Friday
No evenings, weekends, or on-call responsibilities.
Pay: Competitive Salary – $125,000 to $135,000
Compensation is determined based on a combination of experience, certifications, interview outcome, and final decision between the client and candidate.
Requirements to Apply
- Doctor of Pharmacy (PharmD) degree from an accredited college of pharmacy
- Active, unrestricted California Pharmacist license
- Current immunization certification preferred
- Minimum one (1) year of clinical pharmacist experience preferred
- Experience in ambulatory care, geriatrics, primary care, managed care, or community-based healthcare preferred
- Strong knowledge of medication therapy management, chronic disease management, and medication safety
- Excellent communication, patient education, and interdisciplinary collaboration skills
- Bilingual Spanish preferred

We are seeking a Clinical Pharmacist to provide comprehensive medication management services while collaborating with an interdisciplinary healthcare team. This role focuses on optimizing medication therapy, improving clinical outcomes, educating participants and caregivers, and supporting high-quality, patient-centered care for medically complex older adults.

Responsibilities
- Perform comprehensive medication reviews and medication reconciliation
- Optimize medication therapy through evidence-based clinical recommendations
- Monitor medication safety, effectiveness, and potential drug interactions
- Provide medication counseling and education to participants and caregivers
- Collaborate with physicians, nurses, and interdisciplinary team members on individualized care plans
- Support chronic disease management through medication optimization
- Participate in quality improvement and medication safety initiatives
- Maintain accurate clinical documentation within the electronic medical record
- Ensure compliance with state, federal, and organizational pharmacy standards
- Promote participant safety and positive health outcomes through comprehensive pharmaceutical care
