Pay
$75,000 – $85,000 annually
Compensation is determined based on a combination of experience, certifications, interview outcome, and final decision between the client and candidate.
Potential incentive opportunities may be available.
Requirements to Apply
- Bachelor’s Degree in Marketing, Communications, Public Relations, Business, Healthcare Administration, or related field preferred
- Minimum two (2) years of experience in healthcare marketing, community outreach, business development, public relations, or senior services
- Experience working with seniors, caregivers, healthcare providers, or community organizations preferred
- PACE, Home Health, Hospice, Managed Care, Senior Living, Healthcare Outreach, or Community Relations experience strongly preferred
- Valid California Driver’s License and current automobile insurance required
- Current CPR/BLS and First Aid certification required
- Must obtain DHCS Marketing Representative Certification within 90 days of hire
- Strong public speaking, networking, and relationship-building skills
Job Summary
Talent Strategies is partnering with a respected healthcare organization supporting a PACE (Program of All-Inclusive Care for the Elderly) program serving seniors throughout the San Fernando Valley.
We are seeking a Community Outreach Specialist to serve as a key ambassador within the community, helping connect older adults and their families to comprehensive healthcare services that support independent living and improved quality of life.
This role is ideal for a relationship-builder who enjoys being out in the community, developing partnerships, educating referral sources, and driving program awareness and growth. Responsibilities
- Develop and maintain relationships with healthcare providers, senior centers, community organizations, faith-based groups, and referral partners
- Conduct community presentations, educational workshops, health fairs, and outreach events
- Promote awareness of PACE services throughout the community
- Generate referrals and support enrollment growth initiatives
- Serve as a resource for seniors, caregivers, and community partners seeking healthcare solutions
- Conduct preliminary conversations with prospective participants and families
- Coordinate referral handoffs and collaborate with enrollment teams
- Maintain accurate outreach activity and referral tracking documentation
- Assist with development and execution of outreach and marketing strategies
- Analyze outreach effectiveness and identify opportunities for growth
- Support community engagement, branding, and public awareness initiatives
- Participate in community events, networking opportunities, and organizational outreach activities
- Maintain compliance with all applicable healthcare marketing and outreach regulations

- Frequent local travel throughout the San Fernando Valley and surrounding service areas
